Between 2010 and 2013, an attempt was made to integrate 13 providers with over 100 mental health and addiction services into one new integrated agency – the Algoma Anchor Agency. This newly created agency was formed and incorporated in 2012, but late in 2013 the concept “died.” Find out why the concept arose, the steps taken to create the agency and the reasons for its’ demise. Explore the lessons learned and the positive and negative consequences experienced by those affected, including the providers and the recipients of services.
This presentation highlights the Palliative Care Community Team, an interdisciplinary team of healthcare professionals that provide clinical and non-clinical community-based care to palliative and end-of-life patients. This collaboration of seven cross-sectoral organizations focuses on opportunities to create a broader, comprehensive, seamless service model for Scarborough residents to 24/7 quality palliative care through a variety of program services and supports.
The NSM CSS Collaborative is a group of agencies who provide community support services in the NSM LHIN region. These agencies all receive LHIN funding as well as funding from other sources. The agencies provide a wide range of services to adults with ongoing community needs. Services include meals, transportation, disability assistance, supportive housing and more. Agencies range from small to large and some are provincial or national.
Since 1929, Dixon Hall Neighbourhood Services has been the place for east Toronto residents to get the help they need to dream, grow and thrive. Over the past 85 years, their services have grown and changed to support the needs of the vibrant neighbourhoods they serve, including the recent integration (in January 2014) with Mid-Toronto Community Services to ensure strong, effective services for our community seniors.

Inclusion Research unites the framework of Inclusion (developed by Health Nexus) with the experience of listening to women (gained by OWHN through conducting focus groups), and the methodology of training people who are homeless as researchers (pioneered by AMRP).
